What is All-on-4?
All-on-4 is a full-arch dental implant procedure in which an entire row of teeth (upper or lower jaw) is supported by just 4 titanium implants strategically placed in the available bone. The implants are positioned at angles that maximize stability and minimize the need for bone grafting.
Unlike removable dentures, the prosthetic bridge attached to All-on-4 implants is fixed — you don't remove it at night, you don't use adhesives, and it doesn't slip when you eat or speak. Patients consistently describe the transition from dentures to All-on-4 as life-changing.
The procedure was developed in the 1990s and has been refined extensively. Modern All-on-4 has a very high, well-documented long-term success rate. Materials have evolved from acrylic to zirconia and hybrid bridges that look and function nearly identical to natural teeth.

The All-on-4 process step by step

Free consultation with 3D imaging
Dr. Vivian Menendez and the implant team evaluate your case using 3D cone-beam CT imaging. This shows bone density, nerve location and existing tooth structures — critical for surgical planning. You leave the consultation with a written treatment plan and pricing.

Pre-surgical workup
Medical clearance from your physician (if needed for general health conditions), final impressions or scans for the temporary prosthesis, blood work to confirm safe sedation. Any remaining failing teeth are extracted at this stage or at surgery day.

Surgery day
Under sedation (oral or IV), Dr. Vivian places 4 implants in the planned positions — typically 2 vertical in the front and 2 angled toward the back of the jaw. Surgery takes 2-4 hours per arch. Any remaining unhealthy teeth are removed during the same procedure.

Same-day temporary teeth
Before you leave the office, a temporary fixed bridge is attached to the implants. You go home with a complete set of teeth — no toothless period, no removable temporary. The temporary stays in place during healing.

Healing (osseointegration)
Over the following months, the implants fuse with your jawbone (osseointegration). You wear the temporary bridge during this time, eating softer foods initially and progressing to normal diet as healing progresses. We see you periodically to monitor healing.

Permanent prosthesis design
Once healing is complete, we design the final prosthetic bridge. You choose materials (acrylic for budget, zirconia for premium aesthetics and durability). Enrique crafts the final bridge in-house — color, shape and texture customized to your face.

Final placement
The permanent bridge is attached to the implants. From this point forward, you eat, speak and smile like you did with your natural teeth. The bridge stays in place permanently — no removal, no adhesives, no slipping.

Maintenance
You return for routine professional cleanings every 6 months, just like with natural teeth. The prosthetic bridge typically requires no replacement for 10-15+ years. Implant maintenance is straightforward — brushing and flossing as taught.
Recovery and aftercare
Recovery from All-on-4 surgery is typically easier than patients expect. Mild to moderate discomfort for 3-7 days, manageable with prescribed pain medication. Most patients return to non-strenuous work within 4-7 days.
For the first 7-10 days, you eat soft foods (pasta, eggs, fish, smoothies, yogurt). Over the following weeks you progress back to a normal diet. By 4-6 weeks post-surgery, most patients are eating everything they want.
Some swelling and bruising of the face is normal for the first week and resolves on its own. Strict adherence to post-op instructions — cold compresses initially, salt water rinses after 24 hours, no smoking, no straws — significantly reduces complication risk.
The permanent bridge is placed only after osseointegration is confirmed (we confirm it before placing the bridge). During this time you wear the temporary, which feels and functions like real teeth — just with restrictions on extreme hard foods.

You choose between two main options: acrylic (more budget-friendly, lighter weight, may show wear over time) or zirconia (more durable, more natural-looking, stain-resistant, longer lifespan). We discuss both options during consultation and recommend based on your priorities. Hybrid options combining materials are also available.
"Rejection" in the immune sense is essentially impossible — titanium is biologically inert. The implants can, in rare cases, fail to integrate with bone due to infection, insufficient bone density, smoking, or uncontrolled diabetes. We screen for these risk factors during consultation and adjust the plan accordingly. Failed implants are typically replaced successfully on retry.
Yes — once you have the permanent bridge in place (once healing is confirmed), you can eat virtually any food you ate with natural teeth, including steaks, apples, corn on the cob and nuts. The fixed bridge gives bite force similar to natural teeth. During the temporary phase, we recommend softer foods, but progress to a normal diet by 4-6 weeks.
