Art Dental Studio is a dental practice licensed under the State of Florida,

Practice Act) and the ethical guidelines of the American Dental Association (ADA).

Art Dental Studio provides cosmetic, general, orthodontic, and restorative
dental services. All treatments are performed by licensed dental professionals

A formal treatment plan will be provided during your consultation. No guarantee
of specific outcomes is expressed or implied.
3. Appointments, Cancellations and No-Shows
Appointments must be scheduled in advance by phone or through our website

Cancellations require a minimum of 24 hours notice.
Failure to cancel within 24 hours or failure to appear for a scheduled
appointment (no-show) may result in a cancellation fee at the discretion
of Art Dental Studio.
Art Dental Studio reserves the right to reschedule or cancel appointments
due to medical emergencies or circumstances beyond our control.
4. Informed Consent
Before any dental procedure, patients will be required to review and sign an
informed consent form. This form will detail the nature of the treatment,
potential risks, benefits, and available alternatives. Signing the informed
consent form is required before treatment begins.
For patients under 18 years of age, a parent or legal guardian must provide
consent prior to any treatment.
5. Payment and Financial Responsibility
Payment is due at the time of service unless prior arrangements have been made.
Art Dental Studio accepts major credit cards, debit cards, and approved
financing options.
Patients are responsible for verifying their insurance coverage prior to
treatment. Art Dental Studio will assist in filing insurance claims but is
not responsible for unpaid insurance balances.
Unpaid balances may be subject to collection procedures in accordance with
Florida law.

— is the exclusive property of Art Dental Studio and is protected under United
