With your written authorization: We will share your PHI with other
healthcare providers only when you have provided explicit written consent.
With insurance companies: To process claims on your behalf, as authorized
by you during the intake process.

6. Cookies and Website Tracking
Our website uses cookies — small text files stored on your device — to improve
your browsing experience. We use the following types of cookies:
Essential cookies: Required for the website to function correctly.
These cannot be disabled.
Analytics cookies: Used to understand how visitors interact with our
website. Data collected is anonymized and aggregated.
Preference cookies: Used to remember your location preference and
other settings.
You may disable non-essential cookies through your browser settings at any time.
Disabling cookies may affect the functionality of certain parts of our website.
We do not use cookies to serve advertising on this website.
We do, however, use measurement tools that may share information with third-party
platforms, and we would rather say so plainly than leave you with a promise we
cannot keep:
Our website loads Google Tag Manager and Google Analytics. Google receives
your IP address, the pages you visit and how you reached us.
Our tag manager is capable of loading additional measurement or advertising tags —
including Meta and TikTok — without a change to the website's code. Which tags are
active at any given time is a setting, not something visible in the page itself.
We maintain a server-to-server connection that can forward appointment-request
details to TikTok's advertising platform, with the email address and phone number
converted into a code (hashed) rather than sent in the clear. Its purpose is to match
a request to the campaign that brought you here.
If you would prefer not to be included in this, you can disable non-essential cookies in
your browser and use the opt-out controls that Google, Meta and TikTok each provide.
7. Data Security
Art Dental Studio implements administrative, physical, and technical safeguards
to protect your personal and health information, including:
Encrypted data transmission (SSL/TLS) on our website
Secure storage of patient records in HIPAA-compliant systems
Access controls limiting PHI access to authorized personnel only
Regular security assessments of our digital systems
In the event of a data breach affecting your personal information, we will
notify you as required by the Florida Information Protection Act (FIPA)
and HIPAA Breach Notification Rule within the legally required timeframes.
8. Data Retention
We retain patient health records for a minimum of 7 years from the date of
the last treatment, or until a minor patient reaches the age of 18 plus 4
years, whichever is longer — in accordance with Florida Administrative
Code Rule 64B5-17.002.
Website contact form submissions are retained for a maximum of 12 months
unless a patient relationship is established, in which case they become
part of the patient record.
9. Children's Privacy
Our website is not directed at children under the age of 13. We do not
knowingly collect personal information from children under 13 through
our website. If you believe a child under 13 has submitted information
through our website, contact us immediately at the information below
so we can delete it promptly.
For patients under 18, all consent and privacy authorizations must be
provided by a parent or legal guardian.
10. Changes to This Privacy Policy
We reserve the right to update this Privacy Policy at any time to reflect
changes in our practices or applicable law. When we make changes, we will
update the "Last updated" date at the top of this page. We encourage you
to review this policy periodically.
